How to claim NDIS resources from The Sensory Kids Outlet

 

Our Provider Registration Number is 4050027276

We are currently approved to provide supports in ACT, NSW, QLD, SA, Tas and Victoria.

We are waiting approval for WA and NT.

 

NOTE: We can only process NDIS claims for products if the funds are in the following Support Areas:

  • Assistive Products Personal Care/Safety
  • Assistive Equipment for Recreation

Please contact NDIS to discuss your plan if you require equipment under other categories

 

HOW TO ORDER (Plan Managed)

Please follow the steps below to use your allocated funding to purchase products:

  1. You or your Plan Manager download and complete our NDIS Service Agreement.
  2. Return Service Agreement to us by email admin@sensorykidsoutlet.com.au or text a picture of the form to Kiele at  0407 988 484
  3. Complete your shopping online and select “Direct Bank Transfer” under payment options OR send us detailed email of what you would like to purchase.
  4. Ensure your contact details and delivery address are included.
  5. Enter “NDIS Plan Managed” in the comments section with Plan Manager’s details.
  6. We will process the order and when payment has been received, products will be shipped.

 

HOW TO ORDER (Self Managed)

If Your Plan states “NDIS will pay you directly for these supports” or you are Self-Managed

TO ORDER PRIOR TO SUBMITTING PAYMENT REQUEST TO NDIS:

  1. Download and complete our NDIS Service Agreement and return it to us by email admin@sensorykidsoutlet.com.au or text a picture of the form to Kiele at  0407 988 484
  2. Add all items to the cart, include your contact and shipping details and in the comments section select “NDIS Self Managed”.
  3. Select your method of payment.
  4. When payment has been received, products will be shipped.
  5. We will email you an invoice which you may use to complete your NDIS Payment Request through the NDIS Portal.

TO ORDER AND THEN SUBMIT PAYMENT REQUEST TO NDIS:

  1. Download and complete our NDIS Service Agreement and return it to us by email admin@sensorykidsoutlet.com.au or text a picture of the form to Kiele at 0407 988 484
  2. Add all items to the cart, include your contact and shipping details and in the comments section select “NDIS Self Managed Payment Pending”.
  3. Select your method of payment as Direct Bank Transfer.
  4. We will email you an invoice which you may use to complete your NDIS Payment Request through the NDIS Portal.
  5. When you have received your funding, contact us with credit/debit card details to pay over the phone, or pay by direct bank transfer.
  6. When payment has been received, products will be shipped.
